PET Scan for Lung Nodule: What It Means and What to Expect

A lung nodule is a small, rounded area that appears on a chest X-ray or CT scan. Although most lung nodules are not cancerous, some require additional testing to better understand what caused them and whether treatment is needed.

A positron emission tomography, or PET, scan may be recommended to evaluate how active a lung nodule is and whether it has features that could be associated with cancer.

Understanding how a PET scan works, what the results may mean, and what to expect during the procedure can help you feel more prepared for the next steps.

What Is a Lung Nodule?

A lung nodule is a small spot or growth in the lung that appears on an imaging test, such as a chest X-ray or CT scan. It may also be called a pulmonary nodule. Most lung nodules are discovered by accident when a person has imaging for another reason, such as a cough, chest pain, pneumonia, shortness of breath, an injury, or lung cancer screening.

The term “nodule” is generally used when the spot measures 3 centimeters or less. An abnormal area larger than 3 centimeters may be called a lung mass and usually requires a more detailed evaluation.

Many lung nodules are benign. Possible causes include old infections, scar tissue, inflammation, healed granulomas, and noncancerous growths. However, some nodules can represent early-stage lung cancer.

Doctors evaluate lung nodules by considering their size, shape, density, location, rate of growth, and appearance on imaging. They also consider symptoms and personal risk factors, such as age, smoking history, prior cancer, and exposure to substances that can increase the risk of lung disease.

What Is a PET Scan?

A PET scan is an imaging test that measures activity within the body. PET stands for positron emission tomography. When the test is used to evaluate a lung nodule, it is often combined with a CT scan in a procedure known as PET/CT.

The CT portion provides detailed images of the body’s structures, including the size, shape, and location of the nodule. The PET portion shows areas that are using more energy than expected.

Cancer cells often use more energy than normal cells, which may cause them to appear more active on a PET scan. However, infection, inflammation, and healing tissue can also show increased activity. This is why a PET scan can provide important clues but cannot diagnose cancer on its own.

During a PET/CT examination, the CT images are typically obtained along with PET images so the areas of increased activity can be matched to specific structures within the body.

Why a PET Scan May Be Used for a Lung Nodule

A PET scan may be recommended when a lung nodule has features that require further investigation. It is most commonly considered when a nodule is large enough for PET imaging to provide useful information, particularly when it measures approximately 8 millimeters or larger.

Nodules larger than 8 millimeters may be evaluated with follow-up CT imaging, PET/CT, biopsy, surgical removal, or a combination of these approaches. The best option depends on the nodule’s appearance, the likelihood of cancer, the person’s overall health, and whether the nodule has changed over time.

A PET scan may help doctors:

  • Estimate how suspicious a lung nodule appears
  • Determine whether a biopsy may be necessary
  • Identify an appropriate area for biopsy
  • Help plan surgery or other treatment
  • Look for abnormal activity in nearby lymph nodes
  • Check for suspicious areas elsewhere in the body
  • Decide whether monitoring or additional testing is appropriate

Note: A PET scan is usually not the first test used to identify a lung nodule. The nodule is typically discovered and measured with a chest CT scan first. PET imaging may then be used when additional information is needed.

Does Every Lung Nodule Need a PET Scan?

No. Most lung nodules do not automatically require a PET scan. Many small nodules are managed with follow-up CT scans instead. This is especially common when a nodule is tiny, smooth, stable, or found in someone with a relatively low risk of lung cancer.

PET scans may not provide reliable information about very small nodules because the scanner may not be able to detect activity accurately within such a small area. As a result, a tiny nodule may appear inactive even if it still requires monitoring.

Doctors may decide not to use PET imaging when:

  • The nodule is very small
  • The nodule has features that strongly suggest a benign cause
  • The nodule has remained stable for a long period
  • The person has a low risk of lung cancer
  • A follow-up CT scan is the more appropriate next step
  • A biopsy or surgery is already clearly indicated
  • Infection or inflammation is strongly suspected

Note: The decision is based on the individual situation. A PET scan is one of several tools that may be used to evaluate a lung nodule, but it is not necessary for every patient.

What Size Lung Nodule Usually Needs a PET Scan?

PET scans are generally more useful for larger nodules. They are often considered when a pulmonary nodule measures approximately 8 millimeters or more because smaller nodules can be difficult to evaluate accurately with PET imaging.

This does not mean that every nodule larger than 8 millimeters must undergo a PET scan. Rather, PET becomes a more reasonable option at this size when the nodule’s CT appearance, growth pattern, or the person’s risk factors raise concern.

A nodule larger than 8 millimeters may be evaluated with PET imaging if it is:

  • Newly discovered
  • Increasing in size
  • Irregularly shaped
  • Spiculated
  • Part-solid
  • Located in an area associated with a higher cancer risk
  • Found in someone with a current or previous smoking history
  • Found in someone with a history of cancer
  • Not clearly explained by infection, inflammation, or scarring

Note: A nodule smaller than 6 millimeters is often too small for PET imaging to be useful. Nodules between 6 and 8 millimeters are frequently monitored with follow-up CT scans first, although the exact plan depends on the clinical situation.

How a PET Scan Works

Before a PET scan, a small amount of radioactive tracer is injected into a vein. The tracer most commonly used for cancer imaging is fluorodeoxyglucose, or FDG, which is similar to glucose.

Cells that use more glucose may absorb more of the tracer. Many cancer cells have increased metabolic activity and use glucose more rapidly than normal cells, causing them to appear brighter or more active on the scan. This increased activity is often described as tracer uptake.

After the tracer is injected, you will usually rest quietly while it circulates through your body. You will then lie on a table that moves through the PET/CT scanner. The scanner detects areas where the tracer has collected and creates images for the medical team to review.

The CT images help identify the precise location and structure of the nodule, while the PET images show its metabolic activity.

What Does It Mean If a Lung Nodule “Lights Up”?

When someone says that a lung nodule “lights up” on a PET scan, it means the nodule has absorbed an increased amount of the tracer. This increased uptake may raise concern for cancer, but it does not prove that cancer is present.

A lung nodule may show increased activity because of:

  • Lung cancer
  • An active infection
  • Inflammation
  • A granuloma
  • Sarcoidosis
  • Recent pneumonia
  • Healing tissue
  • Other active noncancerous conditions

A PET scan cannot always determine why a nodule is active. It can indicate that further evaluation may be needed, but it cannot replace a tissue diagnosis when the cause remains uncertain.

If a nodule shows increased uptake, the next step may involve a biopsy, surgical removal, follow-up imaging, or treatment for a suspected infection. The recommendation depends on the nodule’s size, appearance, level of activity, and the person’s overall risk.

What Does It Mean If a Lung Nodule Does Not Light Up?

If a lung nodule does not show increased activity on a PET scan, the result may be reassuring. However, it does not completely rule out cancer.

A nodule may not light up because:

  • It is benign
  • It is too small to evaluate accurately
  • It is a slow-growing cancer
  • It is a ground glass or low-activity nodule
  • Its activity is below the scanner’s detection range

Some lung cancers absorb relatively little tracer, particularly when they are slow-growing or appear as ground glass nodules. For this reason, a negative PET scan does not always mean that follow-up imaging can stop.

The result must be interpreted together with the CT findings, the nodule’s size and appearance, its growth over time, and the person’s risk factors.

What Is SUV on a PET Scan?

A PET scan report may include a measurement called the standardized uptake value, or SUV. This number estimates how much tracer has collected in a particular area.

A higher SUV may make a nodule appear more suspicious, but there is no single SUV value that can confirm or exclude cancer. Infection and inflammation can cause high uptake, while some cancers may have relatively low uptake.

Doctors therefore do not rely on SUV alone. They also examine the pattern of activity, the CT appearance, the size and location of the nodule, nearby lymph nodes, previous imaging, and the person’s medical history.

Note: If your report includes an SUV measurement, ask your healthcare provider to explain how the value relates to the rest of your findings.

PET Scan Results: Common Possibilities

A PET scan report may describe the amount of tracer uptake within the nodule. It may also discuss nearby lymph nodes and any areas of increased activity elsewhere in the body.

No Significant Uptake

This means the nodule did not show a meaningful increase in metabolic activity. The finding may be reassuring, particularly when the nodule is large enough to be evaluated reliably and does not have suspicious features on CT.

Follow-up CT imaging may still be recommended if the nodule is small, ground glass, part-solid, irregular, or otherwise concerning.

Mild Uptake

Mild uptake means the nodule shows some activity but not a strongly abnormal amount. This can occur with infection, inflammation, healing tissue, or certain tumors. Depending on the full clinical picture, the next step may involve follow-up CT imaging, another PET/CT scan, a biopsy, or treatment for a possible infection.

Moderate or Intense Uptake

Moderate or intense uptake may raise greater concern for cancer, especially when the nodule is solid, growing, irregular, or spiculated. However, infections and inflammatory conditions can also cause strong uptake. Additional evaluation is commonly needed and may include a biopsy, surgical consultation, or further imaging.

Uptake in Lymph Nodes

If nearby lymph nodes show increased activity, doctors may need to determine whether the finding is related to cancer, infection, inflammation, or another condition. A lymph node biopsy may be recommended because it can help establish a diagnosis and determine whether cancer has spread beyond the original nodule.

Uptake Elsewhere in the Body

A PET/CT scan may identify increased activity outside the lungs. These findings can affect the evaluation and treatment plan, particularly when cancer is suspected.

However, not every active area represents cancer. Normal organs, inflammation, infection, muscle activity, and benign conditions may also absorb the tracer. Additional imaging or biopsy may be necessary to understand an unexpected finding.

PET Scan vs. CT Scan for Lung Nodules

CT and PET scans provide different types of information.

A CT scan shows detailed anatomy. It helps doctors evaluate the nodule’s size, shape, borders, density, location, and relationship to nearby structures. It is also the primary imaging test used to determine whether a nodule has grown over time.

A PET scan measures metabolic activity. It helps doctors determine whether a nodule, lymph node, or another area is using more glucose than expected. The tests are often combined because the structural information from CT and the metabolic information from PET can provide a more complete picture.

Note: A PET scan does not replace CT monitoring in every case. Some nodules require repeat CT scans even when PET imaging has already been performed.

PET Scan vs. Biopsy

A PET scan can indicate whether a lung nodule appears more or less suspicious. A biopsy can provide a sample of tissue that can be examined under a microscope. This is the primary difference between the two tests. PET provides imaging information, while biopsy may provide a definite diagnosis.

A lung nodule biopsy may be performed using:

  • A needle inserted through the chest wall
  • Bronchoscopy through the airways
  • Navigational bronchoscopy
  • Robotic bronchoscopy
  • Surgery

A CT scan may be used to guide a biopsy needle toward a suspicious area within the lung. Bronchoscopy may be used when the nodule can be reached through the airways or when nearby lymph nodes need to be sampled.

A PET scan may help determine whether a biopsy is needed and identify the most appropriate area to sample. For example, if a nearby lymph node shows significant uptake and is easier to reach than the lung nodule, doctors may biopsy the lymph node first.

PET Scan vs. Lung Cancer Screening

Lung cancer screening is performed with a low-dose CT scan, not a PET scan. Screening is intended to detect possible lung cancer before symptoms develop in people who have a higher risk of the disease.

A PET scan is usually considered only after a concerning nodule or another abnormality has already been found. It is not used as a routine lung cancer screening test.

If a nodule is discovered during screening, the next step may involve repeat CT imaging, PET/CT, biopsy, or referral to a specialist. The plan depends on the size and appearance of the nodule and the person’s individual risk factors.

When a PET Scan May Be Less Reliable

PET scans can provide useful information, but they have important limitations. The results may be less reliable in certain situations.

Very Small Nodules

Very small nodules may be below the scanner’s reliable detection range. A small cancer may not show noticeable activity simply because there are not enough active cells for the scanner to detect.

Ground Glass Nodules

Ground glass nodules may show little or no uptake, even when they require continued evaluation. Some early or slow-growing lung cancers appear as ground glass nodules and may not absorb much tracer.

Slow-Growing Cancers

Some cancers grow slowly and have relatively low metabolic activity. These tumors may produce a negative or only mildly abnormal PET result.

Infection and Inflammation

Infections and inflammatory diseases can absorb the tracer and appear suspicious on PET imaging. This may lead to a false-positive result, in which the scan raises concern even though the abnormality is not cancerous.

High Blood Sugar

Because FDG is related to glucose, high blood sugar can interfere with tracer uptake and affect image quality. This is why patients are usually given instructions about eating, drinking, exercise, and diabetes medications before the examination.

False Positives and False Negatives

A false-positive result occurs when the PET scan appears suspicious for cancer even though the nodule is benign. This may happen with infections, inflammation, granulomas, sarcoidosis, or healing tissue.

A false-negative result occurs when the PET scan does not show concerning activity even though cancer is present. This may happen when the nodule is very small, ground glass, part-solid, or slow-growing.

These limitations are why doctors do not base major decisions on PET findings alone. The results are interpreted alongside CT characteristics, symptoms, risk factors, previous imaging, and biopsy findings when available.

How to Prepare for a PET Scan

Your imaging center will provide specific instructions before the examination. It is important to follow them carefully because food, exercise, blood sugar, and certain medications can affect the way the tracer is distributed throughout the body.

Common preparation instructions may include:

  • Avoid eating for several hours before the scan
  • Drink plain water as instructed
  • Avoid strenuous exercise before the examination
  • Tell the medical team if you have diabetes
  • Follow instructions regarding medications
  • Wear comfortable clothing
  • Remove metal objects when instructed
  • Tell the team if you are pregnant or breastfeeding
  • Bring previous imaging studies if requested

Note: People with diabetes may receive additional instructions about blood sugar control, meal timing, insulin, or other medications. Do not change your medication schedule unless your healthcare team specifically instructs you to do so.

What Happens During the PET Scan?

When you arrive at the imaging center, a member of the medical team may check your blood sugar. The radioactive tracer is then injected into a vein.

After the injection, you will usually rest quietly while the tracer circulates through your body. Talking, walking, chewing, or moving excessively may affect where the tracer collects, so you may be asked to remain still and relaxed.

During the scan, you will lie on a table that moves slowly through the PET/CT scanner. You will need to remain still so the images are clear. The procedure is not usually painful, although some people may find it uncomfortable to lie in one position.

Note: The entire appointment may take a few hours because of the preparation and tracer uptake period. The actual imaging portion is usually shorter than the full visit.

Is a PET Scan Safe?

A PET scan uses a small amount of radioactive tracer, while the CT portion uses X-rays. These expose the body to radiation, but the amount is generally considered acceptable when the test is medically necessary.

The potential benefit of obtaining important diagnostic information is weighed against the small risk associated with radiation exposure. Unnecessary repeat imaging should still be avoided.

Tell the healthcare team before the scan if you are pregnant, think you may be pregnant, or are breastfeeding. Special precautions or a different imaging plan may be necessary.

Allergic reactions to the PET tracer are uncommon. However, you should tell the staff about any allergies or previous reactions to medications, contrast material, or imaging procedures.

What Happens After the PET Scan?

Most people can return to their normal activities after the examination unless the imaging center provides different instructions.

You may be encouraged to drink fluids to help your body eliminate the tracer. The radioactive material loses its activity over time and is removed primarily through the urine.

The images will be reviewed by a physician trained in interpreting PET and CT studies. A report will then be sent to the healthcare provider who ordered the test.

PET scan reports often contain technical terms that can be difficult to understand. Rather than drawing conclusions from individual words or numbers, ask your healthcare provider to explain how the findings relate to your nodule and the rest of your medical history.

What Happens If the PET Scan Is Positive?

A positive PET scan means the lung nodule showed increased tracer uptake. This finding may raise concern for cancer, but it does not provide a definite diagnosis.

Depending on the situation, the next step may include:

  • Biopsy of the lung nodule
  • Biopsy of a nearby lymph node
  • Surgical removal
  • Repeat imaging
  • Treatment for a suspected infection
  • Referral to a pulmonologist
  • Referral to a thoracic surgeon
  • Referral to an oncologist when cancer is strongly suspected or confirmed

Note: The most appropriate next step depends on the nodule’s size, appearance, location, and level of activity. Doctors also consider whether the nodule can be biopsied safely and whether the person is healthy enough to undergo surgery or another procedure.

What Happens If the PET Scan Is Negative?

A negative PET scan means the nodule did not show significant tracer uptake. This can be reassuring, but it does not always end the evaluation.

Further management may include:

  • A follow-up CT scan
  • Continued monitoring over time
  • A biopsy when CT features remain suspicious
  • Referral to a specialist when the overall risk remains concerning

A negative PET scan is generally more reassuring when the nodule is large enough to be evaluated accurately and has low-risk features on CT.

It is less reassuring when the nodule is very small, ground glass, part-solid, irregular, or increasing in size. These nodules may require continued monitoring or additional testing even when PET imaging is negative.

What Happens If the PET Scan Is Unclear?

Some PET scan results are neither clearly positive nor clearly negative. The report may describe mild uptake, nonspecific activity, or findings that could be caused by infection, inflammation, or cancer.

When the result is unclear, the next step may include a short-term follow-up CT scan, another PET/CT examination, a biopsy, or treatment for a suspected infection followed by repeat imaging.

Indeterminate results can be frustrating, but they are not unusual. The goal is to choose the safest and most appropriate next step based on all available information rather than making conclusions from a single scan.

PET Scan for Ground Glass Lung Nodules

Ground glass nodules can be difficult to evaluate with PET imaging. Some show little or no tracer uptake even when they represent a condition that requires monitoring or treatment.

This may occur because ground glass nodules often contain fewer metabolically active cells than solid nodules. Some also grow very slowly, making them less likely to appear active on a PET scan.

A persistent ground glass nodule may still require follow-up even when the PET result is negative. Additional evaluation may be needed if the nodule grows, becomes denser, develops a solid component, or changes in another concerning way.

Note: For these nodules, careful comparison of CT scans over time is often especially important.

PET Scan for Spiculated Lung Nodules

A spiculated lung nodule has irregular or star-like edges extending into the surrounding lung tissue. This appearance can be more concerning than a smooth, rounded nodule, although spiculation does not confirm cancer.

If a spiculated nodule is large enough, a PET scan may be used to determine whether it has increased metabolic activity.

A spiculated nodule that shows strong uptake may require a biopsy or surgical evaluation. However, a negative PET scan does not always eliminate concern. Follow-up imaging or tissue sampling may still be recommended based on the nodule’s size, growth, appearance, and the person’s risk factors.

PET Scan for Multiple Lung Nodules

When several lung nodules are present, a PET scan may help identify whether one nodule is more active than the others. It may also show whether nearby lymph nodes or other areas of the body have increased activity.

Multiple lung nodules can occur for many reasons. Possible causes include previous infections, active infections, inflammation, autoimmune conditions, benign growths, or cancer.

The pattern, size, location, and appearance of the nodules are important. PET findings must therefore be interpreted together with the CT images, medical history, symptoms, and any previous imaging studies.

Risk Factors Doctors Consider Along With PET Results

PET scan results are only one part of the lung nodule evaluation. Doctors also consider personal and imaging-related risk factors when deciding what the findings may mean.

Important risk factors include:

  • Current or previous smoking
  • Older age
  • A personal history of cancer
  • A family history of lung cancer
  • Radon exposure
  • Asbestos exposure
  • Secondhand smoke exposure
  • Workplace exposure to harmful dusts or chemicals
  • Growth of the nodule over time
  • Spiculated or irregular borders
  • Part-solid density
  • An upper-lobe location

Note: The same PET finding may be interpreted differently depending on the person’s overall level of risk. Mild uptake in a low-risk person with a stable nodule may be approached differently from mild uptake in a person with a growing, spiculated nodule and a long smoking history.

Questions to Ask Your Doctor

If a PET scan is recommended for your lung nodule, asking questions can help you understand the purpose of the examination and what may happen afterward.

Helpful questions include:

  • Why is a PET scan being recommended?
  • How large is my lung nodule?
  • Has the nodule changed since my previous scan?
  • What features make the nodule concerning?
  • Is the nodule solid, ground glass, or part-solid?
  • What information are you hoping to obtain from the PET scan?
  • What would it mean if the nodule shows increased uptake?
  • What would it mean if the PET scan is negative?
  • Could infection or inflammation affect the result?
  • Will the scan evaluate nearby lymph nodes?
  • What are the possible next steps after the scan?
  • Will I still need a biopsy?
  • Will I need additional CT scans?
  • When and how will I receive the results?

Note: These questions can help clarify how the scan fits into the overall evaluation and reduce some of the uncertainty surrounding the procedure.

When to Seek Medical Care Promptly

Most lung nodules do not cause symptoms. However, certain symptoms should be evaluated promptly because they may indicate an active lung condition or another serious medical problem.

Contact a healthcare provider if you develop:

  • Coughing up blood
  • New or worsening shortness of breath
  • Persistent chest pain
  • Unexplained weight loss
  • A cough that does not improve
  • Repeated episodes of pneumonia
  • An ongoing fever
  • New or persistent hoarseness
  • Severe fatigue accompanied by other concerning symptoms

Note: Seek emergency medical care for severe difficulty breathing, intense chest pain, fainting, confusion, blue lips or skin, or coughing up a large amount of blood.

How to Think About PET Scan Results Without Panicking

Feeling anxious while waiting for PET scan results is understandable. Many people assume that a positive scan automatically means cancer or that a negative scan means there is nothing more to worry about. In reality, PET results require careful interpretation.

A positive PET scan means the nodule has increased metabolic activity. That activity may be caused by cancer, but it can also occur with infection, inflammation, granulomas, or healing tissue.

A negative PET scan may be reassuring, particularly when the nodule is large enough to evaluate accurately and has no other suspicious features. However, PET imaging may miss very small, ground glass, part-solid, or slow-growing cancers.

An unclear result may simply mean that additional monitoring or testing is needed before the cause can be determined.

The most useful question is not whether the PET scan is positive or negative by itself, but how the PET findings fit with the CT appearance, previous scans, symptoms, medical history, and personal risk factors.

Final Thoughts

A PET scan for a lung nodule can provide valuable information, particularly when a nodule is approximately 8 millimeters or larger or has features that raise concern.

The scan looks for increased metabolic activity, which may occur with cancer but can also result from infection or inflammation. A negative result can be reassuring, but it does not always rule out cancer in small, ground glass, part-solid, or slow-growing nodules.

PET findings should be interpreted together with CT results, changes over time, symptoms, risk factors, and biopsy findings when tissue testing is needed. Follow the recommended evaluation plan and ask your healthcare provider to explain how the results apply to your specific situation.
